New Customers

NEW CUSTOMERS

Business: (Domestic)
  • If you are a business, and want to become a new customer, a New Customer Form (click link) is required.  You may also call us at 1-800-626-1887 or e-mail sales@innovatcorp.com and request a New Customer Form. 
  • You may include it with your board or you may e-mail it back.  Once your board and form are received, we will contact you for a credit card#.
  • If you anticipate future orders, you may also request a Credit Application (click link) or submit one of your own.  3 favorable references are required for an open account.
Business (International)
  • International customers are welcome.  We appreciate the opportunity to do repairs for you.
  • Terms are Credit Card or Electronic Funds Transfer (EFT / ACH) for International Customers.
  • Call us at 1-800-626-1887 or e-mail sales@innovatcorp.com and request a New Customer Form or click link for a New Customer International Form.
  • Once your board and form are received, we will contact you for a credit card #.
  • Unless the Postal Service is requested for return shipping, you will need to provide your Fed-Ex or UPS shipper #.  If you use DHL, you will have to schedule a pickup and e-mail us a shipping label.
For Individuals:
  • Thank you for allowing us to do repairs for you.
  • A New Customer Form is not required.
  • Send in your p.c. board and include your name, address, phone #, and a brief symptom and we will contact you for a credit card # when your board arrives.
  • You may also prepay with a check or money order.  Include an extra $ 20 for return shipping & handling.
